Position Overview
This role is responsible for providing accurate, complete, and appropriate coding using ICD-10-CM/PCS and CPT classification systems for the Richmond and Connersville locations. The coder assigns appropriate APC groupings according to diagnoses documented by treating providers and in accordance with regulatory requirements. The position also abstracts relevant clinical and demographic information from the medical record.
Responsibilities
- Code outpatient medical records for quality planning and assessment, reimbursement, research, and compliance with federal regulations using ICD-10-CM and CPT-4 classification systems.
- Perform data entry and abstraction on patients into the applicable computer system.
- Calculate proper APC assignments.
- Assist providers with appropriate diagnosis determination as needed.
- Stay current with coding guidelines, rules, regulations, and new codes.
- Refer coding and system questions to the Coding Manager, Supervisor, and/or Director in a timely manner.
- Work assigned edit work queues to determine correct coding and modifier assignments.
- Assist with abstraction and retrieval of data for special studies.
- Assist other departments with coding functions as needed.
- Code encounters in accordance with productivity and quality standards established by the organization.
- Follow all laws, regulations, and guidelines related to federal and state programs, with emphasis on preventing fraud, waste, and abuse.
- Perform other duties and projects as assigned to support department goals and outcomes.
- Participate in area and department meetings and attend other meetings as requested.
Requirements & Qualifications
Qualifications
- High School Diploma or GED required.
- One year of previous same-day surgery/observation coding experience preferred.
- Certification as a Certified Coding Specialist (CCS), Certified Professional Coder (CPC), Registered Health Information Technician (RHIT), Registered Health Information Administrator, or similar certification required.
