Position Summary
Analyzes patient records for completeness of documentation, including missing signatures or dictation, according to regulatory guidelines and hospital policies. Prior to document imaging, prepares paper documents in discharged medical records for scanning, verifies patient identification on all sheets, and uses document imaging and processing applications to scan, index, and perform quality assurance so documents are transferred to the electronic record in Meditech.
Core Job Functions
- Reviews scanned documents and indexes them to the appropriate category if the barcode does not read.
- Reviews documents for missing signatures and assigns them to the appropriate physician for e-signature.
- Creates batch cover sheets, repairs records for scanning, applies patient identification labels if needed, arranges information in chronological order, and adds thinned pages.
- Monitors image quality while scanning and rescans documents as needed.
- Answers phones promptly using good customer service skills and follows confidentiality policies.
- Assists physicians with locating information and provides guidance on dictation systems or e-signatures when requested.
- Monitors receipt of records from all locations, checks discharge lists, locates missing records, and maintains tracking logs.
Requirements & Qualifications
Education
- Associate degree in Health Information Administration preferred.
Experience
- 1–2 years of medical records or electronic records processing preferred.
- Customer service skills required.
License and Certification
