Inpatient Coder

Rochester General Hospital

Review clinical documentation and diagnostic results to assign appropriate ICD-10-CM and ICD-10-PCS codes for billing, internal and external reporting, research, and regulatory compliance.

This role supports reimbursement optimization, resolves regulatory edits, identifies billing error patterns, and assists with workflow improvements to reduce billing errors.

Location: Remote (Riedman)

Requirements & Qualifications

Qualifications

  • One of the following certifications is required: CCS, CCS-P, CPC, CPC-H, CMC, RHIT, RHIA, CPC-A, or a specialty coding certification.
  • Associate degree in Health Information Management may be accepted, with RHIT exam required within 1 year of hire.
  • At least 2 years of progressive coding experience in a hospital or multi-specialty physician practice setting preferred.
  • Full CPC certification must be obtained within 24 months if hired with CPC-A.
  • For HOMECARE roles, HCS-D certification is required within 16 months of hire.

Responsibilities

  • Perform detailed review of inpatient record documentation to assign diagnosis and procedure codes using ICD-10-CM and ICD-10-PCS.
  • Meet departmental productivity guidelines with 95% accuracy.
  • Use Care Connect, UDS, and Clintegrity systems to obtain ICD-10 codes and DRG assignment.
  • Formulate compliant physician coding queries when documentation is unclear or incomplete.
  • Update POA indicators, Point of Origin, Discharge Disposition, and other required data fields.
  • Manage workflow edits and technical issues related to coding AR days.
  • Correct failed claim errors, misclassified accounts, and audit findings in a timely manner.
  • Attend coding team meetings and training sessions.
  • Provide assistance to physicians and clinical quality staff regarding documentation opportunities and reimbursement issues.
  • Abide by AHIMA Standards of Ethical Coding and comply with organizational policies and official coding guidelines.
