The Medical Coder supports the financial and operational integrity of the organization by reviewing clinical documentation, assigning diagnosis and procedure codes, ensuring compliance with payer guidelines and regulatory standards, and working with providers and revenue cycle teams to promote accurate documentation, reduce denials, and support timely reimbursement.

Core responsibilities

  • Review and analyze patient medical records to assign diagnosis and procedure codes using ICD-10, CPT, and HCPCS classification systems.
  • Ensure coding accuracy and compliance with payer policies, federal and state regulations, and documentation standards.
  • Collaborate with providers and clinical staff to clarify documentation and obtain additional information as needed.
  • Submit coded encounters to the billing system to support insurance claim processing and reimbursement.
  • Conduct internal coding audits and validations to identify discrepancies, reduce denials, and strengthen revenue cycle performance.
  • Maintain current knowledge of coding guidelines, payer requirements, and regulatory updates.
  • Perform other duties as assigned.
Requirements & Qualifications
  • Strong working knowledge of medical terminology, anatomy, and physiology.
  • Demonstrated attention to detail and a high level of accuracy in coding, documentation review, and data entry.
  • Proficiency in medical coding software, electronic health records (EHR) systems, and related databases.
  • Strong technical and data management skills.
  • Current Certified Professional Coder (CPC) certification through a recognized organization such as AAPC.
