As a Home Health Coder II, you will review and code home health and hospice records using clinical documentation such as H&P, discharge summaries, progress notes, and face-to-face notes. You will document coding recommendations, OASIS edits, and work with team leads and denial teams to resolve coding-related issues.
Key responsibilities include:
- Review and code from clinical notes
- Maintain or exceed productivity and quality standards
- Work with the A/R team on coding-related denials and rejections
- Maintain current knowledge of coding guidelines and federal regulations
- Support professional growth through ongoing education and training
- Review OASIS time points and interdisciplinary documentation in the EMR for timely locks and CMS submission
- Use available tools and develop training strategies to improve outcomes
- Address SHP alerts and reports related to outcomes, adverse events, and utilization in coordination with the Quality Support Specialist and/or Team Lead
Requirements & Qualifications
Required qualifications:
- High school diploma or GED preferred
- Minimum 2 years of professional fee coding experience required
- At least 1 year of coding experience in home health and hospice specialties
- Relevant education may substitute for experience requirement
- Knowledge of medical terminology, anatomy and physiology preferred
- Knowledge of pathophysiology preferred
- Coding certification required
- OASIS certification preferred but not required
