As a Home Health Coder II, you will review and code home health and hospice records using clinical documentation such as H&P notes, discharge summaries, progress notes, and face-to-face notes. You will document coding recommendations, OASIS edits, and support resolution of coding-related issues in collaboration with team leads, denials teams, and A/R staff.
You will help maintain productivity and quality standards, stay current on coding guidelines and federal regulations, and contribute to workflow accuracy in the EMR to support timely OASIS locks and CMS submission. The role also supports quality improvement efforts by addressing SHP alerts and reports in coordination with quality support and team leadership.
Required:
- High school diploma or GED preferred
- Minimum 2 years of professional fee coding experience
- At least 1 year of coding experience in home health and hospice specialties
- Coding certification required
- Knowledge of medical terminology, anatomy and physiology preferred
- Knowledge of pathophysiology preferred
