Primary responsibility is the assignment of ICD-10 and CPT codes for all inpatient discharges and outpatient records in a timely and accurate manner.
Duties & Responsibilities
- Accurately assign ICD-10 diagnosis and procedural codes on all inpatient discharges and outpatient records using the 3M Code-finder in a timely manner.
- Ensure patient information is correctly abstracted in the CPSI information system within seven days of patient discharge.
- Enter and sequence ICD-10-CM diagnosis and procedure codes properly.
- Correctly indicate procedure dates and procedural physician with the procedure code.
- Calculate DRGs for all Medicare inpatient discharges.
- Verify discharge disposition reflects the physician discharge order on the medical record.
- Confirm admission type code is correct.
- Ensure service is correct.
- Enter physicians correctly, listing the attending physician first and subsequent physicians after.
- Assemble charts and deficiencies according to hospital guidelines.
- Assist others as needed or as directed by the Medical Records Director.
- Keep the Medical Records Director informed of any problems or concerns that delay or compromise the coding process.
Working Conditions
- Work is performed in a well-lighted, well-ventilated, air-conditioned, and heated office.
- Minimal exposure to bloodborne pathogens, body fluids, or potentially infectious material.
- Prolonged sitting with light physical work.
- Requires ability to hear, communicate, and maintain good visual acuity.
- Minimal lifting requirements up to 25 pounds.
- Must have good use of upper extremities.
Requirements & Qualifications
Education, Training, and Experience
- Knowledge of medical terminology, anatomy, and computer/typing skills.
- Experience is preferred but not required.
- Ability to decipher physician and nurse orders.
