The HIM Coder Analyst I reviews and interprets patient medical record documentation to identify diagnoses and procedures, then assigns ICD-10-CM/PCS and CPT-4 codes accurately and timely to the highest level of specificity based on physician documentation. The role focuses primarily on emergency department and outpatient clinic coding and may assist with ambulatory surgery simple cases.
The position abstracts specified information from the patient medical record and enters data into the electronic health record system for billing and company reporting. The role requires maintaining a minimum coding accuracy rate of 95% or above.
The coder communicates with physicians and other providers regarding documentation requirements and collaborates with Clinical Documentation Specialists to ensure coding and documentation integrity. The position also requires staying current on coding, documentation, rules, and guideline changes.
Candidates should be able to work independently with minimal guidance, maintain confidentiality, and demonstrate strong attention to detail, organization, communication, critical thinking, and problem-solving skills.
One year of current and continuous full-time ICD-10 and CPT-4 coding experience.
Technically competent and fluent in using electronic health record applications, automated encoders, and other required software and hardware.
Proficiency in Microsoft Office Excel and Word.
Demonstrated coding knowledge and proficiency through an on-site skills assessment with a passing score of 90% accuracy prior to hire.
Preferred certifications:
- RHIA
- RHIT
- CCS
- CPC
Candidates with AHIMA credentials must provide current continuing education records.
If not currently credentialed, candidates must meet one of the following:
- Experienced coders: provide documentation of extensive coding experience and coursework in anatomy, pathophysiology, medical terminology, ICD-10-CM, and CPT-4; obtain CCA within 6 months and CCS within 12 months of hire.
- New graduates: provide proof of completion of an AHIMA- or AAPC-accredited program within the last 3 months; be eligible to sit for a national certification exam; obtain the applicable certification within 3 months of graduation.
