Director of MDS (RN)

A brand-new skilled nursing and rehabilitation facility in Johns Creek, Georgia is seeking an experienced Director of MDS to build and lead the resident assessment program from the ground up. This leadership role reports to the Director of Nursing and is responsible for owning the Resident Assessment Instrument process, ensuring accurate and timely MDS 3.0 assessments, and supporting reimbursement, compliance, and quality outcomes.

The position offers the opportunity to establish assessment workflows, the triple-check process, and interdisciplinary routines in a new facility, with support from nursing leadership, ownership, and operations as the census grows. The role is on-site and plays a key part in clinical care, documentation, and revenue integrity.

Requirements & Qualifications

Required Qualifications

  • Active, unrestricted Registered Nurse license in Georgia
  • Graduate of an accredited nursing program (ADN or BSN)
  • Working knowledge of the RAI process, MDS 3.0, Care Area Assessments, and interdisciplinary care planning
  • Understanding of PDPM, Medicare Part A/B, and Medicaid case-mix reimbursement in a SNF/LTC setting
  • Current BLS/CPR certification
  • Proficiency with EMR and MDS software
  • Strong organizational, analytical, and communication skills
  • Ability to pass background check, fingerprinting, drug screen, TB test, and required immunizations

Preferred Qualifications

  • RAC-CT certification or willingness to obtain
  • 2+ years of MDS Coordinator or MDS Director experience in a nursing home or SNF
  • Experience opening or building an MDS/assessment program in a new facility
  • Hands-on experience with PointClickCare or similar clinical/MDS platforms
  • Knowledge of Georgia Medicaid case-mix and PDPM optimization strategies
  • Prior charge nurse, unit manager, or nursing leadership experience
