Case Manager role with Encompass Health in inpatient rehabilitation.
Join a nationally recognized rehabilitation provider and serve as a vital link in each patient's care journey. In this role, you will manage resources, coordinate patient care from admission through discharge, and help guide interdisciplinary plan-of-care decisions.
You will work closely with the care team to support patient goals, improve discharge outcomes, monitor utilization and payer-related issues, and ensure accurate and timely documentation throughout the case management process.
License or certification
- Must be qualified to independently complete an assessment within the scope of practice of the applicable discipline (for example: RN, SW, OT, PT, ST, or Rehabilitation Counseling)
- If licensure is required in the state, an active license is required
- Must meet eligibility requirements for CCM® or ACM™ certification upon entry or within two years
- CCM® or ACM™ certification required, or must be obtained within two years of placement in the Case Manager II position
Minimum qualifications
- For nursing: Associate Degree in Nursing required; RN licensure required; BSN preferred
- A diploma is acceptable only in states where it meets licensure or certification requirements
- For other eligible licensed or certified healthcare professionals: bachelor's degree required; graduate degree preferred
- 2 years of rehabilitation experience preferred
