Accurately uploads and indexes outside records using applicable software, routing documentation to clinicians as appropriate. Creates or selects the correct patient, encounter, and/or order and assigns the proper document type and description when indexing into the EHR.
Incorporates internal and external clinical documentation from electronic and paper sources into the EHR through work queues, rounding, eDelivery, or importing, while meeting quality, accuracy, and productivity benchmarks.
Analyzes medical records for missing documentation and signatures, assigns or edits record deficiencies in the chart management system, and supports clinicians with record completion activities.
Collaborates with data integrity and other functional areas to correct documentation errors and escalate quality or privacy concerns when needed.
Ensures scanned documents are clear, legible, and usable by end users, and serves as a point of contact for record completion support.

- High school diploma or equivalent
