Remote community-based clinical coordinator/care manager role supporting Upper Peninsula Health Plan members throughout the Eastern Upper Peninsula counties.
This position provides care management services to assess, plan, and coordinate medical and supportive services across the continuum of care for select members. The role requires frequent in-person visits with members in homes, nursing facilities, and other care settings, plus periodic travel to the organization’s headquarters in Marquette, Michigan.
Responsibilities include member assessment, care planning, coordination with medical providers and community organizations, monitoring service delivery, identifying quality and risk concerns, participating in process improvement efforts, and supporting compliance with clinical and regulatory standards.
Education
- Licensed registered nurse required
- Preferred: Bachelor of Science in Nursing, limited licensed bachelor/master of social work, or fully licensed bachelor/master of social work
- Must be licensed in the state of Michigan
Experience
- Minimum of 2 years of clinical or health-related experience as a licensed registered nurse or social worker
- Preferred: 2 years of clinical managed care experience or 5 years of clinical experience as a licensed registered nurse or social worker
- Preferred experience in care management and reviewing statistical data
Other requirements
- Valid driver's license with proof of insurance
- Working vehicle
- Keyboarding proficiency
- Working knowledge of Microsoft Office Word and Excel
- Strong communication, organization, and prioritization skills
- Ability to interpret and analyze data
- Working knowledge of Microsoft Access and PowerPoint is desired
