Medical Records Technician (Health Information Technician)

This position is located in the Health Information Management (HIM) section of Health Administration Service (HAS) at the VA Northern Indiana Healthcare System. Medical Records Technicians (Health Information Technicians) perform incomplete health record analysis, review and abstract health record information, and monitor documentation for accuracy, completeness, timeliness, and regulatory compliance.

Key responsibilities

  • Perform quantitative and qualitative reviews of health record documentation for regulatory reviews and deficiency analysis
  • Review health records to ensure sufficient information for medico-legal requirements, continuity of patient care, education, and research needs
  • Identify, track, and follow up on health record deficiencies
  • Track unsigned and uncosigned notes and communicate deficiencies to clinical staff
  • Expedite completion of health records for transfers, legal requests, regulatory reviews, and reimbursement purposes
  • Resolve filing errors on reports uploaded by a transcription vendor
  • Verify and correct statistical data related to medical transcription
  • Compare admission, discharge, transfer, and outpatient records with transcription logs for timeliness and accuracy
  • Serve as a Dragon Local Administrator, including account setup, support, troubleshooting, training assistance, and usage reporting
  • Work with interdisciplinary staff to resolve documentation and completion issues
  • Use EHR tools to analyze and monitor documentation and data capture deficiencies
  • Collect, interpret, and trend data from audits and validation activities
Requirements & Qualifications

Basic requirements

  • U.S. citizenship required
  • Proficiency in written and spoken English
  • Background/security investigation required
  • Pre-employment physical examination required
  • Seasonal influenza vaccination participation required
  • Must complete all application requirements

Qualifications

  • One year of creditable experience with knowledge of medical terminology, anatomy, physiology, and health records, with at least six months providing the KSAs needed for MRT HIT work
  • Or an associate's degree in health information technology/management or a related field with qualifying coursework
  • Or an equivalent combination of education and experience
  • May qualify under the VA grandfathering provision for current VHA employees

Grade-level expectations

  • GS-04: Meets basic requirements
  • GS-05: One year of experience equivalent to the next lower level or qualifying bachelor's degree; must demonstrate communication, software, records management, prioritization, and medical terminology knowledge
  • GS-06: One year at the next lower level; must demonstrate navigation, analysis, error correction, guideline application, and follow-up skills
  • GS-07: One year at the next lower level; must demonstrate judgment, communication, software use, trend analysis, independent work, and quality assessment abilities
