Health Records Technician 2

Western State Hospital is seeking a Medical Record Department Specialist with experience working across multiple health records systems. The role supports medical record management, discharge analysis, coding, release of information, records retention, and customer service for internal and external stakeholders.

Key duties include:

  • Conduct routine and required audits of physician and medical staff documentation for legibility, completeness, timeliness, and authentication.
  • Perform complete medical record reviews for discharge records to ensure accuracy and completeness, including ICD-9 and ICD-10 diagnosis coding.
  • Support The Joint Commission (TJC), Centers for Medicare & Medicaid Services (CMS), and Office of Inspector General (OIG) audits for compliance with standards and WSH policies.
  • Assemble and complete discharge analysis and reanalysis of medical records.
  • Code diagnoses using ICD-9-CM, ICD-10-CM, and DSM IV-R coding manuals.
  • Index diagnoses and maintain patient records in the WSH database system.
  • Coordinate hospital admissions and ensure accurate diagnostic coding and required paperwork.
  • Process public records disclosures and protected health information requests, including redaction and documentation.
  • Assist medical staff and management with research related to medical records.
  • Assist with training RHIT students during clinical work.
  • Support records retention scheduling, archiving, and destruction in compliance with legal requirements.
Requirements & Qualifications
  • Bachelor's degree in health information management, business administration, or a related field, plus 3 years of experience in a Health Information Management office, office management, healthcare privacy office, compliance office, or closely related area.
  • Or an associate degree in health information management, business administration, or a related field, plus 5 years of relevant experience.
  • Current and ongoing AHIMA Registered Health Information Technician (RHIT) credential.
  • Expert knowledge of CMS, TJC, and other regulatory standards related to health records management.
  • Strong customer service skills.
  • Experience communicating orally and in writing with medical professionals, legal professionals, and the public.
  • Excellent attention to detail, organization, and interpersonal skills.
