Family Health Centers of San Diego (FHCSD) is a large federally qualified health center serving San Diego County with a mission to provide caring, affordable, high-quality health care and supportive services to everyone.
This Health Information Management Specialist II role supports medical records operations, patient chart preparation, release of information, records requests, and quality assurance/data abstraction activities. The position also provides front-line customer service, supports clinical staff, and helps maintain accurate documentation and workflow across electronic and paper record systems.
Key responsibilities include:
- Preparing patient charts for upcoming visits by reviewing open orders, incomplete documentation, follow-up needs, open referrals, and pending results
- Coordinating release of medical information according to department and regulatory guidelines
- Verifying authorizations, processing requests, and communicating with requestors
- Reviewing medical records for accuracy and completeness
- Participating in quality assurance, quality control, and data abstraction work
- Generating billing statements and recording billing activities
- Supporting HIPAA and privacy compliance for PHI sharing
- Linking and closing related orders and encounters when obtaining records
- Communicating findings and potential risk management issues to leadership
- Working across internal and external systems, applications, and workflows
- High school diploma or GED required
- Medical Terminology course required or completion within 90 days of orientation
- Medical Assistant certification preferred
- RHIA or RHIT certification preferred
- 1-2 years of HIM services experience in a clinical or hospital setting preferred
- 1-2 years of experience with medical records requests preferred
- 1-2 years of experience with an EHR and practice management system preferred
- Intermediate computer literacy, including Excel, Outlook, Word, Adobe Acrobat, eFax systems, and shared drives
- Intermediate knowledge of medical terminology and record content required
- Strong interpersonal, written, and verbal communication skills
- Strong organizational skills, attention to detail, time management, and ability to meet deadlines
- Ability to work independently and adapt to changing priorities
- Ability to perform accurate data entry at high speed
- Bilingual English/Spanish preferred; Vietnamese, Persian, Farsi, or Somali highly desirable
