The Care Facilitator is a patient's first point of contact and serves as a patient advocate delivering exceptional customer service. This role checks patients in and out of the medical clinic and provides basic secretarial and administrative support.
Key responsibilities include:
- Greeting patients, checking them in and out, and scheduling appointments
- Maintaining lobby cleanliness and organization
- Managing provider schedules and helping ensure patients are scheduled appropriately
- Entering and updating patient and emergency contact information in the system
- Making follow-up calls and sending birthday/greeting cards to patients
- Answering phones and taking accurate messages
- Communicating clinic information to patients and others professionally
- Verifying insurance and collecting copays
- Assisting with specialist referral scheduling
- Escorting patients to exam rooms and helping ensure their comfort
- Supporting HEDIS-related needs and reporting hospitalizations
- Distributing customer service surveys and escalating concerns when needed
- Participating in daily and weekly huddles
- Coordinating transportation-related tasks, including rides and deliveries
- Maintaining confidentiality of patient information and medical records
- Transmitting correspondence by mail, email, or fax
Requirements & Qualifications
- High school diploma or equivalent required
- Graduation from a nationally accredited Medical Assistant program preferred
- Minimum of 1 year of work experience in a medical clinic or similar environment required
- BLS for Healthcare Providers preferred
- Strong customer service skills and a passion for serving others
- Working knowledge of medical insurance and/or EMR systems
- Proficiency with phones, computers, software, and basic IT systems
- Excellent verbal and written communication skills
- Strong listening skills and attention to detail
- Proficiency in Microsoft Office Suite, including Excel, Word, and Outlook
- Ability and willingness to travel locally and regionally up to 10% of the time
- Fluency in English
- Flexibility to work evenings, weekends, and/or holidays as needed
