Pharmacist Specialist - Outpatient Medication Management

The Pharmacy Specialist provides high-quality, cost-effective pharmaceutical care to specific patient populations within the assigned scope of practice. For comprehensive medication management (CMM), the role provides medication management for patients referred by INTEGRIS Medical Group primary care physicians and employees, and their dependents referred by INTEGRIS Health Partners care coordinators.

Core responsibilities include responding to drug information requests, identifying drug use issues, coordinating concurrent drug use evaluation programs, supporting adverse drug event monitoring and reporting, tracking and trending ADRs, preparing formulary review materials, providing in-service education, managing formulary-approved medications, eliciting medication histories, reconciling medication lists, assessing medication regimens, addressing medication-related problems, educating patients and caregivers, and documenting CMM visits in the electronic health record.

Requirements & Qualifications

Required experience:

  • PGY1 residency or equivalent experience

Required education:

  • Graduation from an accredited school of pharmacy
  • Advanced degree in pharmacy and/or pharmacy residency or fellowship, or equivalent experience

Required license/certification:

  • Current Oklahoma Pharmacy license, or ability to obtain licensure within 45 days of hire if licensed in another state
  • Must be able to communicate effectively in English, verbally and in writing

Preferred qualifications:

  • Specialty residency or fellowship in the practice area, if available
  • Board certification in the practice area specialty if available, or BCPS preferred within one year of hire
