Removal of subcutaneous implantable defibrillator pulse generator only

Common Language Description

The procedure described by CPT® Code 0322T involves the removal of a subcutaneous implantable defibrillator pulse generator. This procedure is typically performed under general anesthesia or monitored anesthesia care (MAC), along with local anesthesia to ensure patient comfort. The pulse generator is usually located in the left chest area, necessitating careful preparation and draping of the skin to maintain a sterile environment. The surgical process begins with an incision made directly over the pulse generator, allowing the surgeon to access the device. Once exposed, the pulse generator is meticulously dissected from the surrounding tissue, and the electrode connected to the device is identified and disconnected. Following this, the pulse generator is removed from its subcutaneous pouch. If the procedure does not involve the replacement of the old pulse generator, the pouch is simply closed after removal. However, if a new pulse generator is being inserted, it is connected to the existing electrode and placed back into the pouch. The new device is then programmed externally, and an arrhythmia is induced to ensure proper functionality. Throughout the procedure, the patient is monitored to confirm the device's appropriate response. Finally, the electrode and device are secured, and the incision is closed. This code is specifically used when the pulse generator is removed without replacement, while a different code, CPT® Code 0323T, is designated for cases where the old generator is replaced with a new one during the same surgical session.
