The CPT® Code 27524 refers to the open treatment of a patellar fracture, which involves a surgical procedure aimed at repairing a broken kneecap (patella). This procedure may include internal fixation, which is the use of devices such as pins, screws, or plates to stabilize the fracture, and may also involve a partial or complete patellectomy, where part or all of the patella is surgically removed. Additionally, soft tissue repair is performed to ensure proper healing and function of the knee. The procedure typically begins with obtaining radiographs to confirm the presence of a fracture and conducting a neurovascular examination to assess the integrity of the nerves and blood vessels around the injury site. The surgical approach is usually anterior, allowing for direct access to the patella. During the operation, the surgeon inspects the patella and surrounding structures, removes any loose bodies that may hinder the reduction of the fracture, and manipulates the patella back into its correct position. Following this, internal fixation devices may be applied to maintain stability, or the patella may be excised if necessary. The procedure concludes with the suturing of the quadriceps and patellar tendons, followed by layered closure of the soft tissues and skin. Post-operative care may involve immobilization of the knee to promote healing.
