A colectomy, partial, is a surgical procedure that involves the removal of a segment of the colon, which is part of the large intestine. This operation is performed to address various medical conditions affecting the colon, such as tumors, inflammatory bowel disease, or other significant damage. During the procedure, a colostomy or skin-level cecostomy is created, which allows for the diversion of intestinal contents to an external pouch. The process begins with the careful mobilization of the affected segment of the colon, which involves dividing the embryonic fusion planes and the peritoneum while ensuring that the blood supply to the remaining colon is preserved. The mesentery, which is the tissue that supports the intestines, is also divided to facilitate the removal of the diseased segment. Once the affected portion is excised, the surgeon creates a colostomy or cecostomy by making a small incision in the skin at the designated site. This incision is deepened through the subcutaneous tissue, and fat is excised until the anterior rectus fascia is reached. The fascia is then opened with care to protect the underlying rectus muscle and its blood supply. Blunt dissection is used to separate the rectus fibers, allowing access to the peritoneum, where an opening is made to accommodate the stoma. To prevent postoperative complications such as obstruction, the proximal segment of the cecum or colon is ideally brought through a peritoneal tunnel. Finally, the colon or cecum is everted and sutured to the skin, establishing the stoma for waste elimination.
