Laminectomy, also known as lamina excision, is a surgical procedure aimed at alleviating back pain and relieving pressure on the spinal cord, spinal nerve roots, and/or cauda equina. This procedure is particularly relevant in cases where there is spinal or lateral recess stenosis, which can lead to significant discomfort and neurological symptoms. The lamina is a bony structure that forms the posterior aspect of the vertebral arch, and its removal is essential for accessing the underlying structures of the spine. During the procedure, a posterior skin incision is made over the affected area of the spine, allowing the surgeon to retract the overlying fat and muscle to expose the lamina. The excision of the lamina, along with the paired ligaments known as the ligamentum flavum, facilitates a thorough inspection of the spinal anatomy, including the superior and inferior articular facets and the pars interarticularis. This access enables the surgeon to explore the openings under the facet joints where spinal nerves emerge, and to remove any bone that may be compressing the nerve roots. The procedure also involves enlarging the intervertebral foramen to decompress the spinal cord, lysing any adhesions between the dura mater and the ligamentum flavum, and carefully dissecting the spinal nerve roots and/or cauda equina to free them within the intervertebral foramen. The surgical wound is then closed in layers, ensuring proper healing.
