Reviews medical records after discharge for completeness in compliance with the health facility’s policies, accrediting organization standards, federal and state requirements. Assures that patient abstracts are correctly entered into the medical record deficiency system.
Collects all discharged patient medical records daily, prepares them for scanning by removing staples, placing batch header sheets, and organizing records orderly. Scans and indexes health and administrative information into the electronic health record system within established timeframes, maintaining an error rate of 5% or less.
Combines current patient medical records with previous discharges if applicable. Analyzes records for completeness and accuracy per department and facility policies, monitors deficiencies requiring physician completion, and forwards records to coders.
Coordinates release of medical information to requesters, verifies authorizations following policies and legal requirements, and maintains security and confidentiality regulations.
Performs quality audits to ensure integrity of scanned images. Maintains supply closet and orders supplies as needed.
Supports performance improvement audits and assists with administrative tasks including answering phones and taking messages.
Familiar with pediatric and OB/GYN medical record requirements. Maintains compliance with health facility policies on confidentiality and regulatory standards.
Demonstrates professionalism, adheres to dress code and work schedules, participates in staff meetings, trainings, and continuous quality improvement activities.
Complies with all organizational policies regarding ethical business practices and communicates the mission, ethics, and goals of the health facility and department.
High School graduate or equivalent. Two or more years of similar health facility experience preferred. Knowledge of accrediting organization standards and state medical record requirements. Effective communication skills in English, both verbal and written; additional languages preferred. Basic computer knowledge. Must provide immunization and PPD documentation as required.
