Elevate Health and Rehabilitation is a member of the Ascent Healthcare Management family of skilled nursing facilities and rehabilitation centers in Western North Carolina. They are resident-centered and quality-focused with a workplace culture that uplifts people. This position is for an experienced MDS (Minimum Data Set) Coordinator.

As an MDS Nurse, you will oversee the coordination and participate in completing the Resident Assessment Instrument (MDS, CAA’s and Care Plan) in accordance with Federal and State regulations. You will assist the Interdisciplinary Team in completing assessments, notify team members of assessment schedules and changes, lead or participate in meetings to ensure federal billing requirements are met, collect information from medical records, bedside assessments, staff, residents and family interviews, and develop systems to ensure all team members have completed necessary assessments. You will also observe nursing care, review documentation, make recommendations, and assist with chart audits.

Requirements:

Registered Nurse or Licensed Practical Nurse licensed in North Carolina
Minimum two years clinical experience in healthcare, preferably long-term care
Current/active CPR certification
Personal integrity and professionalism for effective teamwork with care team, patients, and families
Knowledgeable of nursing, medical practices, and regulations pertaining to long-term care
