Primary Care Physician

HouseCall MD is urgently hiring a full-time Primary Care Physician to provide mobile medical care for seniors and medically complex patients across the San Gabriel Valley and nearby Los Angeles areas.

This role involves providing primary care, chronic condition management, post-hospital follow-up visits, and care coordination at patients' homes, assisted living facilities, skilled nursing, and residential care settings.

The physician should be personable, clinically strong, tech-savvy, and able to build trusted relationships with patients, families, facility staff, and care teams.

Support includes centralized scheduling, chart preparation, office support for orders and referrals, a proprietary mobile app for route optimization, AI-integrated EMR system, dictation tools, and optional company driver.

Key responsibilities include managing chronic conditions, conducting post-hospital follow-ups, participating in care coordination, documenting visits, and representing the company professionally.
