The Wound Care Nurse Specialist is a registered nurse providing specialized care for patients with chronic or complex wounds, primarily in palliative and end-of-life care settings. The role focuses on improving quality of life, preventing infection, and promoting healing when possible.
Responsibilities include:
- Assessing and managing wounds, including staging pressure injuries and performing interventions such as cleansing and debridement.
- Maintaining detailed documentation of assessments, treatments, and patient progress.
- Collaborating with a multidisciplinary healthcare team to optimize patient outcomes.
- Educating patients, families, and caregivers on wound care techniques and infection prevention.
- Orienting and educating staff on wound care protocols and updating guidelines.
- Staying current with wound care research, techniques, products, and technologies.
- Providing emotional support to patients and families.
- Recommending evidence-based wound care regimens and coordinating orders.
- Participating in policy development related to wound assessment and treatment, including guidance on ostomies and fistulas.
Skills and Qualifications:
- Ability to work non-judgmentally with diverse populations.
- Strong verbal and written communication skills.
- Ability to work independently and as part of a team.
- Strong organizational and prioritization skills.
- Wound and ostomy assessment and treatment expertise.
- Bilingual ability is a plus.
Work Environment:
- Work occurs in patients' homes, nursing homes, or hospitals contracted by Hudson Valley Hospice.
- Must be able to lift up to 25 pounds.
- Potential exposure to infectious agents and blood/body fluids.
- Requires moderate travel by car and working in varied environments.
Preferred Experience and Education:
- Graduate of an accredited nursing school.
- Minimum two years of nursing experience with hands-on wound care.
- One year of medical-surgical experience preferred.
- Valid New York State nursing license and driver’s license.
- Specialized Wound Care Certification required.
- Certifications such as CWCN, CWON, or WOCN preferred.
Hours: Part-time, 24 hours/week (three 8-hour shifts).
