Schedule & Contract Details:

Start ASAP or within 2 weeks of offer
Location: Grand Rapids, MI
Shift: Monday - Friday 8 AM - 5 PM
Contract: 13 weeks to start with high chance of extension
Hours: Productivity model with 30 units per week
Territory: Holland to Muskegon
On-call: As needed
Compensation: Up to $115,500 per year plus mileage reimbursement and full benefits

Responsibilities include completing assessments, creating and implementing care plans, documenting accurately in HCHB & OASIS, and maintaining communication with care teams and leadership while delivering compassionate care.

Requirements:

Minimum 2 years home health experience visiting multiple patients per day
At least 1 year experience within last 3 years
Proficient in HCHB & OASIS (required)
Active RN license in Michigan
