Nurse Practitioner/Physician Assistant

Monogram Health is seeking skilled Nurse Practitioners and Physician Assistants to make a difference in patients' lives by delivering personalized, compassionate medical care primarily for individuals with chronic kidney disease (CKD) and end-stage renal disease (ESRD/ESKD). The Advanced Practitioner is a key member of an integrated care team including registered nurses and social workers, focusing on improving quality of life and slowing disease progression through home and virtual patient care.

Responsibilities include conducting assessments and annual wellness exams in patients' homes and virtually, educating patients and families about available benefits and programs, documenting comprehensive patient records accurately and timely, coordinating care with providers and specialists, prescribing medications and ordering tests, facilitating patient enrollment for educational resources, participating in care team meetings, and working towards improved patient outcomes by reducing avoidable hospitalizations.

Requirements:

  • Active and unrestricted Registered Nurse and Nurse Practitioner or Physician Assistant license
  • Board certification (NP: ANCC/AANP; PA: NCCPA)
  • Current and unrestricted DEA certificate
  • Ability to practice autonomously without direct supervision
  • Access to transportation with valid driver's license and insurance
  • Proficiency with medical instruments, equipment, and computer-based data management systems
  • Effective verbal and written communication skills
  • Knowledge of HIPAA, OSHA, and federal/state regulations
  • Minimum 2 years direct patient care experience
  • Managed Care/IPA/Health Plan experience
  • Experience conducting annual wellness or comprehensive visits virtually or in-home

Bilingual English/Spanish preferred.

This is a remote position within the employing state, requiring occasional travel for training/meetings.
