Duties include conducting thorough patient assessments, diagnosing acute and chronic conditions across specialties such as internal medicine, geriatrics, pediatrics, and emergency medicine. Manage medication administration, perform procedures such as suturing, catheterization, spinal taps, and sterile processing techniques. Coordinate care through case management, discharge planning, utilization review, and work with individuals with disabilities or developmental challenges. Provide telehealth consultations. Collaborate with physicians and specialists in hospital settings such as Level I trauma centers or ICU units for complex cases. Document all patient interactions accurately using CPT and ICD coding standards, ensuring HIPAA compliance. Assist in clinical research initiatives or quality improvement projects aimed at enhancing patient care standards.
Requirements include a valid Nurse Practitioner license with current certification, proven experience in diverse clinical environments including hospital medicine and ICU, strong proficiency with EMR/EHR systems like Epic, Cerner, or Athenahealth. Skills in advanced procedures such as sonography, gastroscopy, venipuncture, and airway management. Critical care experience and knowledge of CPT and ICD-10/ICD-9 coding standards. Ability to work effectively with multidisciplinary teams and excellent patient care skills with compassionate communication. Familiarity with infection control protocols; experience in hospice & palliative medicine or memory/dementia care is advantageous.
Schedule: 3 days per week in the post-acute setting, hospital weekend rounding available.
