Post-Acute and Long-Term Care (PALTC) - Advanced Practitioner (AP)

The Post-Acute and Long-Term Care (PALTC) Advanced Practitioner (AP) is an Advanced Practice Registered Nurse (APRN) or Physician Assistant (PA) who works independently while collaborating effectively with nursing facility leadership and staff. The AP manages comprehensive medical needs of patients in PALTC environments to improve care quality, healthcare experience, reduce hospitalizations, and decrease costs.

Responsibilities include comprehensive physical exams, diagnosing and treating illnesses, prescribing medications, ordering and interpreting diagnostic tests, monitoring health status, providing preventive care, managing chronic conditions, and collaborating with interdisciplinary teams. The AP practices independently within scope, makes clinical decisions, acts as primary provider, and liaises between facility, families, and external professionals.

Ensure compliance with federal and state regulations, maintain accurate medical records, participate in quality improvement, infection prevention, health and safety education, and remain current with licensure and certifications. Strong communication, leadership, technology proficiency including EHR and telemedicine, and capability to work independently and respond in emergencies are essential.

Qualifications: Degree required (BS-PAS/MPAS/DMSc/MSN-NP/DNP), minimum 2 years experience (long-term care preferred), board certification (AANP/ANCC/PA), Arkansas APRN/PA license required. Work is in-person at statewide PALTC facilities.

  • Board certification: AANP, ANCC, or PA
  • Licensed as Arkansas APRN or PA
  • Competency with EHR systems and telemedicine tools
  • Strong clinical decision-making and leadership skills
  • Ability to work independently and respond swiftly in emergencies
  • Excellent verbal and written communication skills
  • Ability to build collaborative interdisciplinary relationships
