The RN MDS Coordinator coordinates and assists with completion and submission of accurate and timely interdisciplinary MDS assessments, CAAs, and care plans according to CMS RAI Manual regulations and in accordance with applicable laws, regulations, and Life Care standards.
Key responsibilities include:
- Coordinate and assist with completion and submission of interdisciplinary, accurate, and timely MDS assessments, CCAs, and care plans
- Report changes in a patient's condition identified by the MDS assessment to the DON
- Provide education to direct care associates regarding updates or changes to CMS RAI Manual or Skilled Nursing Facility regulations that impact documentation
- Assist with review of the Interdisciplinary Comprehensive Care Plan
- Review Final Validation Reports and confirm assessments are completed and accepted into the CMS QIES system prior to billing
- Notify the Business Office when assessments are not ready to bill
- Review CMS reports to identify late assessments and help develop processes to prevent recurrence
- Attend Daily PPS Meetings, Monthly Triple Check, and other meetings as requested
- Perform functions of a staff nurse as required
- Support patient safety, customer service, and interdisciplinary teamwork
Requirements & Qualifications
Education and licensure requirements:
- Associate's or bachelor's degree in nursing from an accredited college or university
- Current active Registered Nurse (RN) license in the applicable state, maintained throughout employment
- Two years of nursing experience
- Geriatric nursing experience preferred
- CPR certification upon hire or obtained during orientation and kept current
Additional requirements:
- Advanced knowledge in field of practice
- Ability to make independent decisions when needed
- Knowledge of applicable laws, regulations, and guidelines governing post-acute care facilities
- Ability to implement and interpret departmental programs, goals, objectives, policies, and procedures
- Ability to maintain confidentiality and professional working relationships
- Strong communication, judgment, and reasoning skills
- Ability to read, write, speak, and understand English
