The role focuses on completing resident assessments, documenting care needs, coordinating admissions, and helping ensure compliance with applicable regulations while supporting smooth transitions into the facility.
Requirements & Qualifications
- Current Registered Nurse (RN) credential in the State of Minnesota
- Strong understanding of the MDS assessment process
- Knowledge of Medicare/Medicaid regulations
- Excellent clinical assessment and documentation skills
- Strong communication and interpersonal skills
- Detail-oriented with strong organizational abilities
- Ability to work independently and collaboratively
