Provides integrated behavioral health services within a primary care setting, including consultation, health behavior assessment and intervention, crisis management, and coordination of care.
Works collaboratively with medical leaders, nursing, and clinic operations to support patient flow, warm hand-offs, and communication across the multidisciplinary care team.
Supports patients with co-occurring medical and behavioral health needs, including chronic disease, substance use concerns, life stressors, and acute mental health conditions.
Provides on-site crisis assessment, triage, and intervention, and coordinates with community behavioral health organizations, hospitals, and public safety officials as needed.
Maintains accurate electronic patient records and follows HIPAA, documentation, billing, and coding procedures for services delivered in primary care behavioral health and specialty behavioral health care models.
Qualifications
- Behavioral health professional license required
- Master's degree preferred
- Ability to assess mental and behavioral health conditions using DSM, PHQ-9, and other relevant screening tools
- Experience working with integrated care teams and multidisciplinary providers
- Strong documentation and coordination skills
