Certified Diabetes Care and Education Specialist (CDCES)

Certified Diabetes Care and Education Specialist (CDCES) in the Clinical Nutrition Department at Upland Hills Health.

This full-time day shift position serves as the Diabetes Self Management Education (DSME) Program Coordinator for the facility. The role oversees planning, implementation, and evaluation of the Diabetes Self-Management Program, provides diabetes education and support to patients and families, and collaborates with physicians, nursing staff, and other interdisciplinary team members to deliver high-quality care.

Additional responsibilities include nutritional assessments, nutritional counseling, participation in community nutrition programs, menu planning, and contribution to organizational and departmental continuous quality improvement activities.

Requirements & Qualifications

Candidates must have:

  • A bachelor’s or advanced degree as a dietitian, registered nurse, pharmacist, physical therapist, or occupational therapist from an accredited school
  • Completion of an ADA-approved dietetics internship or equivalent education/experience
  • Licensure in Wisconsin as a Registered Dietitian, Registered Nurse, Pharmacist, Physical Therapist, or Occupational Therapist
  • Registered Dietitian status with the Academy of Nutrition and Dietetics if applying as a dietitian
  • CDCES certification or CDCES eligibility
  • Knowledge of patient care standards in Wisconsin and standards of the Academy of Nutrition and Dietetics and the American Association of Diabetes Educators
  • Academic preparation and/or experience in caring for persons with chronic disease
  • Preferred experience with the Epic medical record system
  • Beneficial experience with Microsoft Office Suite
