The procedure described by CPT® Code 27488 involves the removal of a knee prosthesis, which may be a total knee prosthesis, along with the use of methylmethacrylate, and may or may not include the insertion of a spacer. This surgical intervention is typically performed when there are complications associated with the prosthesis, such as infection or other adverse effects that necessitate its removal. The operation begins with a long incision that is strategically made over the femur, extending down over the knee and onto the tibia. During the procedure, careful dissection of the soft tissues is essential to protect the surrounding nerves and blood vessels from damage. The surgeon will remove the femoral and/or tibial components of the prosthesis, ensuring that any infected bone and soft tissue are thoroughly excised to prevent further complications. In some cases, a spacer may be inserted into the joint space to maintain the proper distance between the muscles and other tissues surrounding the knee joint, thereby preventing contraction. This spacer is typically made from one or two pieces of bone cement, which may be infused with antibiotics to help combat infection. After the necessary components have been removed and any additional procedures completed, a drain may be placed if required, and the soft tissues are then closed in layers to promote healing.
