Extended ophthalmoscopy is a comprehensive and detailed examination of the internal structures of the eye, specifically focusing on the back of the eye, known as the fundus. This procedure is particularly important for assessing severe conditions affecting the posterior segment of the eye, such as retinal detachment and melanoma. During the examination, the physician utilizes eyedrops to dilate the pupil, allowing for a better view of the retina and other critical components, including the optic disc, choroid, and blood vessels. The examination is conducted using an ophthalmoscope, which resembles a flashlight and is equipped with various tiny lenses that facilitate a thorough inspection of the eye's interior. Additionally, a slit-lamp microscope may be employed to enhance the examination process. The physician meticulously examines the back of the eye and creates a detailed drawing that accurately represents the condition of the retina. This drawing includes standard colors and labels to document the extent of any retinal detachment, the location of holes or tears, areas of traction, vitreous opacities, hemorrhaging, and lesions. Furthermore, specific conditions such as glaucoma require careful identification of features like cupping, disc rim, pallor, slope, and any pathology surrounding the optic nerve. Following the examination, the physician provides an interpretation that summarizes the findings, conclusions, and impressions derived from the examination. It is important to note that CPT® Code 92225 is used for the initial evaluation of disease in each eye examined, while CPT® Code 92226 is designated for subsequent evaluations of established problems that may be worsening or complicated by additional pathology.
