A caloric vestibular test is a diagnostic procedure used to evaluate the function of the vestibular system, specifically the lateral semicircular canals of the inner ear. This test involves the irrigation of the ears with water, which is recorded using a technique called electronystagmography (ENG). The primary purpose of the caloric stimulation is to assess the responsiveness of the vestibular system, which plays a crucial role in balance and spatial orientation. During the test, the patient is positioned in a reclining manner with their head tilted at a 30-degree angle. This positioning is essential as it aligns the lateral semicircular canals in a vertical plane, optimizing the test's effectiveness. Before the irrigation begins, the physician evaluates the presence of spontaneous nystagmus, which is an involuntary eye movement that can indicate vestibular function. The test employs horizontal electrodes placed at the inner and outer aspects of each eye to accurately record eye movements. The caloric stimulation can be performed using either an alternating binaural bithermal protocol, which involves both cool and warm water irrigation in each ear, or a monothermal protocol, which uses only warm water. The expected physiological response to cool irrigation is fast phase nystagmus beats directed away from the stimulated ear, while warm irrigation elicits nystagmus beats towards the stimulated ear. The results of the eye movement recordings are then analyzed and interpreted by the physician, providing valuable insights into the patient's vestibular function. Each irrigation performed during the test is reported using CPT® Code 92543.
