The CPT® Code 29025 refers to the application of a turnbuckle jacket, specifically designed for the body, which includes the head and neck. A turnbuckle jacket is a specialized type of body cast that is constructed in two halves and connected by metal turnbuckles. These turnbuckles are adjusted at intervals to apply tension, which helps to forcibly straighten the spine prior to surgical intervention. Although the use of turnbuckle jackets has diminished significantly in modern practice, they were a common treatment modality until the 1970s. The procedure begins with the application of a stockinette, which is a thin, stretchy fabric that covers the torso and extends over the neck and head as necessary. Following this, padding is placed over the stockinette to provide comfort and protection. A roll of plaster or fiberglass is then prepared by immersing it in water until fully saturated, after which excess water is gently removed. The saturated material is wrapped around the torso, neck, and head, ensuring that it is smoothed and molded into the desired shape. Once the casting material has set and hardened, the cast is carefully split into two halves, and the edges are trimmed for a proper fit. Finally, the turnbuckle device is affixed and tightened at specific intervals to achieve the intended spinal alignment. It is important to note that for a turnbuckle jacket that only covers the body without extending to the head, CPT® Code 29020 should be used instead of 29025.
