A multi-layer compression system is a therapeutic approach utilized primarily for the treatment and prevention of venous ulcers, which are wounds that occur due to improper functioning of the veins in the legs. This system is designed to apply graduated pressure to the affected area, promoting venous return and reducing swelling. The compression system typically consists of two, three, or four layers, each serving a specific purpose in the overall treatment strategy. Various manufacturers offer complete compression systems that are pre-packaged for convenience. When applying a multi-layer compression system, the healthcare provider first inspects the venous ulcer to assess its severity and the underlying venous disease. Based on this evaluation, the appropriate type of compression system is selected. The application process begins with a wound layer that is placed directly over any existing venous ulcers to protect the wound and facilitate healing. This is followed by additional layers, which may include padding to cushion the area, as well as short-stretch and long-stretch layers that provide the necessary compression. For procedures involving the upper arm and forearm, the specific CPT® code to use is 29583. In contrast, if the compression system extends over the fingers and hand, forearm, and upper arm, the appropriate code would be 29584. It is important to note that the compression system is typically changed at regular intervals, often every week, to allow for inspection of the venous ulcer and to assess the healing progress. Each application or change of the compression system should be reported using the appropriate CPT® code to ensure accurate billing and documentation.
