Nasal endoscopy, surgical, ethmoid sinus, placement of drug eluting implant

Common Language Description

An endoscopic procedure known as nasal endoscopy is performed to examine the ethmoid sinus, which is situated between the eyes. This surgical intervention involves the placement of a drug-eluting implant designed to deliver medication directly to the affected area. The procedure may also include surgical debridement, biopsy of sinus tissue, and the removal of polyps, depending on the specific needs of the patient. The primary indications for this procedure are chronic rhinosinusitis (CRS) and inflammation of the paranasal sinuses and nasal passage lining that persists for more than 12 weeks and does not respond to medical management. The goal of the nasal endoscopy and subsequent implant placement is to restore normal sinus ventilation and drainage, thereby alleviating symptoms associated with sinus obstruction. During the procedure, a rigid or flexible endoscope is inserted through the nasal passage and advanced to the ethmoid sinus. If necessary, polyps are excised using a snare, and tissue samples can be collected with biopsy forceps. The drug-eluting implant, which is a synthetic bio-absorbable device containing a steroid compound, is introduced into the ethmoid sinus using a proprietary endoscopic delivery system. Once in place, the implant expands to fit the sinus cavity, helping to stabilize the middle turbinate, prevent obstructive adhesions, and reduce inflammation. The CPT® Code 0406T encompasses the surgical nasal endoscopy of the ethmoid sinus along with the placement of the drug-eluting implant, while the related code 0407T includes additional procedures such as biopsy, polypectomy, or debridement in conjunction with the implant placement.
