Ablative laser treatment, as described by CPT® Code 0491T, is a specialized procedure aimed at addressing open wounds that may be complicated by the presence of inflammatory mediator cells and biofilms. These factors can significantly hinder the healing process by limiting the availability of essential growth factors required for effective collagen remodeling. The non-contact full field and fractional laser ablation therapy is designed to create microthermal zones within the wound area, which serves multiple purposes: it eliminates damaged epithelial cells, disrupts biofilm formations, and stimulates the secretion of growth factors that are crucial for healing and tissue regeneration. Before the procedure, the area to be treated is typically anesthetized using either a topical cream or a local injection to ensure patient comfort. The laser settings are meticulously tailored to the individual patient's needs, with the treatment depth adjusted according to the size and depth of the wound. The application of the laser can be performed in two distinct methods: a full field approach, where the laser is applied uniformly across the entire surface area of the wound, or a fractional ablation technique, which involves a non-overlapping stamping pattern that creates narrow columns of treated tissue. The laser operates at a specific wavelength that is absorbed by the fluid present in the tissue, leading to vaporization of the fluid and resulting in a thin rim of coagulation around the treated area. This process not only aids in the removal of unhealthy tissue but also promotes a conducive environment for healing. CPT® Code 0491T specifically reports the non-contact, full field and fractional laser ablative treatment for open wounds, applicable per day for a total treatment surface area of the first 20 square centimeters or less. For larger areas, CPT® Code 0492T is utilized to report each additional 20 square centimeters, or part thereof.
