1. Indications

Optical coherence tomography (OCT) is indicated for the following conditions and purposes:

• Actinic Keratosis - Used for the early diagnosis of actinic keratosis, a precancerous skin condition caused by sun damage.
• In-situ Squamous Cell Carcinoma - Aids in the identification of in-situ squamous cell carcinoma, which is a type of skin cancer that has not yet invaded deeper tissues.
• Basal Cell Carcinoma - Facilitates the early detection of basal cell carcinoma, the most common form of skin cancer.
• Margin Delineation - Helps delineate the margins of basal cell and squamous cell tumors prior to surgical excision, which can assist in reducing the size of the surgical deficit.
• Depth Measurement - Differentiates between the upper dermis and epidermis to accurately measure the depth of tumors.
• Diagnosis of Inflammatory Skin Diseases - Aids in diagnosing inflammatory skin diseases such as psoriasis and systemic sclerosis.
• Fungal Nail Infections - Assists in the diagnosis of fungal infections affecting the nails.

2. Procedure

The procedure for optical coherence tomography (OCT) involves several key steps:

• Preparation of the Patient - The patient is positioned comfortably, and the area of skin to be examined is identified. The skin may be cleaned to ensure optimal imaging results.
• Application of the Hand-held Probe - A hand-held probe containing a light beam laser is applied to the target area of the skin. This probe is designed to capture scattered light from the tissue surfaces.
• Image Acquisition - As the probe is moved over the skin, it captures the light that is reflected back from the tissue. This scattered light is essential for creating detailed images of the skin's microstructure.
• Image Processing - The captured light is processed through an optical interferometer, which filters and organizes the data to produce high-resolution images of the tissue layers.
• Interpretation of Images - A qualified healthcare professional interprets the images generated by the OCT, assessing the characteristics of any lesions present and determining their significance.
• Reporting - A comprehensive report is generated, detailing the findings from the imaging procedure, including any identified lesions and their characteristics.

3. Post-Procedure

After the optical coherence tomography (OCT) procedure, there are typically no specific post-procedure care requirements, as it is a non-invasive technique. Patients may resume their normal activities immediately following the examination. However, it is essential for healthcare providers to discuss the results of the OCT with the patient, including any necessary follow-up actions or additional diagnostic procedures that may be warranted based on the findings. Continuous monitoring of any identified lesions may also be recommended to ensure timely intervention if changes occur.
