Ablative laser treatment, as described by CPT® Code 0492T, refers to a specialized procedure aimed at enhancing the healing of open wounds. Open wounds often harbor inflammatory mediator cells and biofilms, which can impede the healing process by limiting the availability of essential growth factors necessary for collagen remodeling. The non-contact full field and fractional laser ablation therapy is designed to address these issues by creating microthermal zones within the wound. This process effectively eliminates damaged epithelial cells, disrupts biofilms, and stimulates the secretion of growth factors, thereby promoting collagen remodeling and facilitating healing. Before the procedure, the area to be treated is typically anesthetized using either a topical cream or a local injection to ensure patient comfort. The laser settings are customized based on the specific characteristics of the wound, including its size and depth. During the treatment, the laser can be applied in two distinct methods: a full field approach, where the entire surface area of the wound is treated, or a fractional ablation technique, which involves a non-overlapping stamping pattern that creates narrow columns of treated tissue. The laser operates at a wavelength that is absorbed by the fluid present in the tissue, leading to vaporization of the fluid and leaving behind a thin rim of coagulation. This innovative approach not only aids in wound healing but also enhances the overall effectiveness of the treatment. CPT® Code 0492T is specifically used to report each additional 20 square centimeters, or part thereof, of the total treatment surface area beyond the first 20 square centimeters, which is reported using CPT® Code 0491T.
