The procedure described by CPT® Code 0284T involves the revision or removal of a pulse generator or electrodes associated with a permanent subcutaneous neurostimulator. This procedure may include imaging guidance if necessary. The primary goal is to address issues related to the implantable pulse generator (IPG) and the electrode arrays that deliver electrical impulses to manage pain. During the revision process, the existing incision over the subcutaneous pocket is reopened to access the IPG and the distal ends of the electrode wires. The connections between the wires and the IPG are carefully examined to ensure proper functionality, and any necessary adjustments or revisions are made. If the electrode arrays are found to be ineffective in delivering impulses to the targeted area of pain, they can be repositioned or replaced. In cases where new electrode arrays are required, the old arrays are removed, and new ones are inserted and connected to the IPG. The procedure also allows for the placement of additional electrode arrays at different sites if deemed necessary. Once all components are confirmed to be functioning correctly, the optimal settings for frequency and intensity of the impulses are programmed into the generator. Finally, the IPG is secured back into the pocket, and the incision is closed. In instances where the entire neurostimulation system is being removed, the pulse generator and electrode arrays are carefully extracted, and the incisions are sutured closed.
