The CPT® Code 80354 refers to a laboratory test specifically designed to measure the levels of fentanyl in biological specimens such as urine, serum, or plasma. Fentanyl is classified as a potent synthetic opioid analgesic, known for its rapid onset of action and relatively short duration. This medication is frequently utilized in conjunction with benzodiazepines to facilitate conscious sedation during various medical procedures. Clinically, fentanyl is indicated for the management of acute postoperative pain, as well as breakthrough pain that may occur in patients with cancer or those suffering from bone fractures. Additionally, it is employed in the treatment of chronic pain conditions. Fentanyl can be administered through various routes, including intravenously (under the brand name Sublimaze), intrathecally in conjunction with spinal or epidural anesthesia, via a transdermal patch (known as Duragesic), and orally in several forms such as a lollipop or lozenge (Actiq), buccal tablet (Fentora), or sublingual spray. The collection of a blood sample for this test is performed through a separately reportable venipuncture, while urine specimens can be obtained either by random voiding or catheterization. The testing of urine, serum, and plasma for fentanyl levels is conducted using quantitative liquid chromatography-tandem mass spectrometry, a highly sensitive and specific analytical technique.
